about the program
My health for life is a free, six-month program where you work with a health coach to achieve your health goals.
It is a State government-funded initiative, delivered by an alliance of health organisations, and is designed to help Queenslanders stay well and lessen their risk of developing conditions such as type 2 diabetes, heart disease, stroke, high cholesterol and high blood pressure.
Choosing either small group sessions, or one-on-one phone coaching, you explore topics such as nutrition, physical activity, understanding your personal risk of developing health conditions and how to make healthier choices a part of everyday life.
With the support of your coach, you will also develop an action plan. Whether your goal is to get more active, eat better, stress less, or lose weight, the focus is on building a step-by-step plan to help you achieve your goals over time.

telephone health coaching
Telephone coaching is a more flexible option, allowing you to participate in the program at a time and location that works for you.
You will have six appointments with your health coach over six months. Appointments one to five are held fortnightly, with a 12-week break between sessions five and six where you will put into practice what you’ve learnt so far.
group programs
*We are pleased to announce that we have reactivated face-to-face group sessions in some QLD areas, and are continually monitoring the situation. For the safety and wellbeing of our group participants, we continue to adhere to the government’s recommended COVID-19 safe practices.
Group programs are available across Queensland and are delivered by local health professionals in your community. They offer a positive and supportive environment to share and learn as a group, while working on your own personal health goals.
You will have six sessions with your group over six months. The first session is a one-on-one appointment with your health coach where your coach will gain an understanding of your needs and goals. Sessions run fortnightly and you will have a 12-week break between sessions five and six.
culturally tailored programs
We also offer culturally tailored group programs for:
- Aboriginal and Torres Strait Islanders
- Pacific Islanders
- Mandarin and Cantonese speaking communities
- Vietnamese communities
- Arabic speaking communities.
Our programs for Culturally and Linguistically Diverse Communities (CALD) are delivered by our Healthier Queensland Alliance partner Ethnic Communities Council of Queensland (ECCQ). All CALD programs are delivered by qualified, bilingual multicultural health workers and are tailored to meet the needs of our participants.
