My health for life is a FREE initiative, with qualified Health Coaches who co-design your plan with you to give you the best outcome for your situation. It’s all about making healthier choices and creating healthier habits.
Delivered in six coaching sessions, at a relaxed pace, the initiative is designed to help you discover ways of changing your day-to-day behaviour — that you can live with. In groups or one-to-one, locally, at home or at work, on the phone, online or in culturally adapted sessions, you can take part and reduce your risk of chronic conditions.
With the support of your Health Coach, you will develop an action plan. Whether your goal is to get more active, eat better, feel less stressed, or lose weight, the focus is on building a step-by-step plan to help you achieve your goals over time.

Group Health Coaching in face-to-face sessions is available across Queensland —delivered by local health professionals in your community. They offer a positive and supportive environment to share and learn as a group, while working on your own personal health goals.
You will have 6 sessions with your group over 18 weeks. The first session is a one-on-one appointment with your Health Coach, where they will gain an understanding of your needs and goals. Sessions run fortnightly, with 4 weeks between sessions 5 and 6.
Telephone Health Coaching is a more flexible option, allowing you to participate in the program at a time that works for you.
You will have 6 appointments with your Health Coach over 18 weeks. Sessions 1 to 5 are held fortnightly, with a 4-week break between sessions 5 and 6, when you will put into practice what you’ve learned.
CULTURALLY TAILORED PROGRAMS

Our programs for Culturally and Linguistically Diverse Communities (CALD) are delivered by our Healthier Queensland Alliance partner Ethnic Communities Council of Queensland (ECCQ). All CALD sessions are delivered by qualified, bilingual multicultural health workers and are tailored to meet the needs of our participants.
‘ON TRACK’ SELF-PACED LEARNING
On Track is the online offering in the My health for life initiative, designed for those seeking a short, self-paced learning option to maintain their current health and well-being. The online Health Check will determine if this program option is available to you.

Too many Queenslanders live with high risk factors for chronic disease. Alarmingly, 62% are overweight or obese, 43% don’t get enough physical activity and 36% exceed the guideline for risky drinking.*
The leading cause of death for Queenslanders is heart disease, at 12.8%¹ and 10,000 of us have a stroke each year.² Like the rest of Australia, over 5% of us live with diabetes.³

It has helped me lose a total of 16kg and developed a much more positive and active approach to life in the last 6 months.
