Health Professionals

The My health for life program works in partnership with GPs, providing patients an opportunity to participate in an evidence-based lifestyle modification program in their local community. Facilitators are trained health professionals, including dietitians and exercise physiologists, who have a keen interest in preventive health.

My health for life is a practical extension of the advice given by GPs to their patients, allowing participants to better understand their lifestyle risks and action their health goals. GPs will be kept informed of their patient’s progress, completing the care cycle. Health professionals are a critical part of the My health for life program, and we look forward to building strong, professional relationships while contributing to your patients’ overall care.

Program content includes

  • Understanding your personal risk of chronic disease
  • An understanding of conditions such as type 2 diabetes, heart disease and stroke
  • Understanding how lifestyle choices impact on health
  • Making change in manageable steps
  • Goal setting and action planning for a healthier lifestyle
  • Healthy eating and moving more
  • How factors such as stress and sleep impact on our health
  • Maintaining good health

“As a facilitator, I love it when you see people have a lightbulb moment and they realise this program is a little different and that it could work well for them.

You can almost feel the relief because I’m sure they initially thought we were just going to lecture them and tell them what food to eat. Of course, it is nothing like that.

This program is about behaviour change and overcoming personal barriers that can be blocking you from making better, healthier choices. Most people know what they could be doing to be healthier but are often unsure how to get there or know where to start. It can be overwhelming!

That’s where we help. Everyone in the group is working towards a different goal, and everyone’s journey will be different, yet everyone supports each other.

The other day a man in my group talked about how he didn’t feel he could make life changes without having someone to support him, then it suddenly occurred to him he was already drawing support from the group in our session. That was a good feeling for all of us.”
