What's Involved

My health for life is like having your own personal health coach as the program is tailored to you.

Program participants receive six sessions of personalised health coaching and support over six months. During that time, we explore your health goals – whether it is to lose weight, get more active, eat better or stress less – and work with you to tackle those goals step-by-step.  

The program is delivered in small group sessions in your local area or, if you prefer, in structured sessions over the phone. The choice is yours.

  • Your first session is a one-on-one, personalised appointment to help tailor the program to your needs.
  • Session two, three, four, five and six are either group-based or phone-based, depending what option you chose.
  • In session six we reflect on your progress and how to maintain momentum moving forward.

You also receive tips, tools and useful information to help you on your journey and will gain insight into topics including nutrition, understanding your personal risk of developing chronic disease and how to make healthier choices a part of everyday life.

*My health for life is a free Queensland Government-funded program delivered by the Healthier Queensland Alliance designed to help Queenslanders stay well and lessen their risk of developing conditions such as type 2 diabetes, heart disease and stroke.
